AN ORD! ANCE APPROVING TWO CONTRACTS BETWEEN THE CITY OF ENGLEWOOD,
COLORADO, AND THE STATE DEPARTMENT OF HIGHWAYS, DIVISION OF HIGH-
WAYS, CONCERNING INSPECTION, RATING AND INVENTORY OF BRIDGES ON
THE FEDERAL-AID URBAN SYSTEM AND ON OFF-SYSTEM ROADS.
WHEREAS , the City of Englewood, Colorado, is desirous of
entering into two contracts with the State Department of Highways,
Division of Highways, which requires the City to inventory, inspect
and rate bridges on the urban system and on off-system roads for
the City will be reimbursed 80% of the cost of said work;
NOW, THEREFORE, BE IT ORDAINED BY THE CITY COUNCIL OF THE
CITY OF ENGLEWOOD, COLORADO:
Section 1. That the City of Englewood, Colorado, and the State
Department of Highways, Division of Highways, shall enter into two
contracts for the inspection of bridges on the urban system and on
off-system roads and rate said bridges for sufficiency, copies of
said contracts, consisting of thirteen pages each including exhibits,
are attached hereto and incorporated herein by reference. Said
contracts provide generally that Federal-aid funds allocated for
the inspection , rating and inventorying of bridges on the Federal-
aid urban system and on off-system roads, under the Surface ·
Transportation Assistance Act of 1978, 23 u.s.c., Section 144, as
amended, will be used to reimburse local agency (City) at the
ratio of 80% Federal-aid funds to 20% local agency funds on costs
eligible for Federal participation, that the total cost of said
work is $3,510 of which the City's share is $702.
Section 2. That the City Council of the City of Englewood,
Colorado, hereby authorizes the Mayor of the City of Englewood to
subscribe his name to said contracts for and in behalf of the City
Council and the City of Englewood and the Director of Fina~ce, ex
officio City Clerk-Treasurer, attest the same.

WHEREAS, pursuant to the Surface Tran sportation Assistance Act
of 1978, 23 U.S.C. Section 144, as amended (23 CFR 650.401) cer-
tain Federal 'funds have been allocated for the inspection, rating
and inventory of bridges on the Federal-aid urba n system and on
off-system roads, hereinaf.ter referred to as the work; and
WHEREAS, pursuant to the aforesai d laws and re g ulations, bridges
on the urban system and off-systems are eligible to be inspected,
rated and inventoried under the program at no cos t tp the State;
and
WHEREAS, Federal-aid funds in the amount of $ 2 443 .00
have been made available for a bri dge inspection project, identi-
fied as BRM 0000(13) consisting of the wo rk on 6 urban system
bridges in Englewood, Colorado
; and
WHEREAS, the State has recei v ed a letter of intent from the
Local Agency, wherein the Local Ag ency h as concurred in its
portion of the project; and
WHEREAS, based on such concurrenc e, the Sta te has prepare d a
D.O.H. Form 463 setting forth initial dcsien d ata and including
stimates of the cost of this project, which d nta and estimates
have been approved by the Local Agency; and
WHEREAS, the matchine ratio for this Federa l -a id projC'ct is
80.007. Federal-aid fund s to 20.00% Local Aecncy funds, it being
understood th at such r a io applies only to such costs ns arc

WHEREAS, the Local Agency has estimated the tota l co st: of
said work to be $ 3 ,054.00 and is prepare d to match said
Federal-aid with its share in the amount of $611 .00 as
evidenced by an ap~ropriate ordinance or r esol ution duly passed
and adopted by the authorized representative s of the Local Agency,
a copy o{ which is attached hereto and made a part hereof; or in
lieu of an appropriate ordinance or resolu tion , the execution of
this contract by its authorized representative(s) establishes
the authority lUlder which the Local Agency ente rs into this con-
tract; and
WHEREAS, the Lo cal ABency intends to engap,e the services of a
consultant to perform the work required Wld er the project; and
WHEREAS, this contract is executed by the State under authorif :~:
of Sections 43-1-102, 43-1-106, 43 -2-101(4)(c) and 43-2-144, C.R.S.
1973, as amended; and
WHEREAS, the parties hereto desire to agree upon the division
of responsibilities with regard to this project.
NOW, THEREFORE, it is hereby a g reed tha t:
A . The State will provi de liaison with the Local Agency
through the State's Bridge Engineer, located at 4201
E. Arkansas Av e., Denver, CO 80222 . Sa id Bridge
Engineer will also be responsible for coordinating
the State's activities under this contract.
B. The Local Agency may enter into a contract with a con-
sultant to do a ll or any por ion of Lh work under the
proposed project; provid d, however, that when Fcderal-
aid funds provided herein ar to par icipatc in the cost
o f work don e by a consultant, the contract shall comply
with the requirements of FHPM, Volum e 1. Chapter 7,
Section 2, Administration of NProtint~d Contrncts.

The contract with the consultant
shall be conditioned upon an acceptable proposal sub-
atantially in accordance with "Scope of Work" (Exhibit A)
attached hereto and made a part hereof and may not be
awarded or executed until the se1ection of the consultant

the State. All changes in the ~ontract shall be by a~p-
plemental written agreement and muat have prior approval
of the State •. Ac. soon as the contract with the consultant " "·
has been awarded, 3 copies of the executed contract shall
be submitted to the State. Any supplemental agreements
shall be similarly submitted.
C. The Local Agency shall be responsible for the adminis-
tration of the consultant's work for the project. with
periodic inspections by the State and Federal Highway
Administration (FHWA). Administration sliall .include.
but not be limited to, monitoring the field ins'pections,
ratings and inventory data, prepared by the Loca~ Agency!s
consultant, for accuracy and completeness.
D. The State will administer the Local Agency c a d Federal-
aid funds for this project in accordance with the
followine provisions :
Costs incurred by the Local Agency
The Local Agency will prepare and submit monthly
charges for the costs incurred relative to the
project. However, bill\ng for less than $5,000
shall not be submitted wtless it is for the final
project payment . The Local Agency 1rrill prepare
project charges in accordance with the Statis
standard policies and procedures. The State will
reimburse the Local Agency for the Federal-aid
share of the project charges following the State's
review an d approval of such charr.cs. Charges

F. The term of thi s contract shall continue through the com-
plction and final ac.ceptance of this project by the State,
FHWA and Local Agency .
· G. During the performance of all work under this contract,
the parties hereto agree to comply with Title VI, of the
Civil Rights Act of 1964, the salient points of which
are show n in th e Non-discrimination Provisions attached
hereto and made a part hereof.
H. The Special Provisions attached hereto arc hereby made ii

BRIDGE INSPECTION, RATING, & INVENTORY
OF
CITY & COUNTY BRIDGES
Scope of Work
General: This work shall include all personnel, materials ancJ equipment to inspect,
rate, and inventory bridges that are located in the city or county. Special equip-
ment such as a hydraulic operated bridge in"'p ect'ion platfonn, when required. will
be paid for as stipulated herein. Bridge inspection reports shall be prepared and
furnished as specified in the scope of work.
Bridge Survey: A review of any existing bridge survey data such as .inspection re-
ports, plans, etc. shall be conducted at the beginning of the inspection program
with city/county officials. A numbering system for bridges, based on guidelines
furnished by the State, shall be established for the city or county. This numbering
system shall be approved by the State. The contractor shall develop a systematic
• plan to search out and inspect all bridges within the area assigned. A copy of
this plan shall be submitted to the State for information.

A county map shall be prepared that identifies each bridge by number and locates
each bridge to the nearest 1/10 mile from some identifiable highway feature.
Bridge Inspection: All bridges and approaches shall be given a thorough inspection
to provide information for the inspection report. The physical inspection shall
be considered part of the work of this contrcct. Where critical structural com-
ponents are inaccessible due to high water, inspections shall be re-scheduled to
accomplish the work. If it is detennined that critical structural components are
not accessible with typical inspection equipment, as determined by the contractor
and concurred in by the State, arrangements shall be made to obtain special in-
spection equipment. A supplemental agreement will be required prior to the con-
tractor proceeding with the ~ork •

The Contractor will be reimbursed for the actual cost of renting special inspection
equipment, operator's salaries, permits or incidental work involved in the procure-
ment or oper•tion of the equipment •

The items to be inspected and reported for Oft-SYSTEM bridges are outlined in the
Structure Inventory and Appraisal (SI&A) Abridged (Cities and Counties), DOH Form
o. 421. The ftells to be inspected and reported for urban system bridges are out-
lined in th~ Structure Inventory and Appraisal, DOH Form No. 422.
The Departllent of Highways Coding Manuals, Structure Inventory Coding Guide for
Bridges Not On The Federal-Aid System and Structure Inventory Coding Guide, elaborate
on each item. All inspection work will be conducted in accordance with the Bridge
Inspectors Trcfnfng Manual 70 •
A minimum of two color photographs will be required for each bridge, one viewing the
top of the bridge looking down the roadway, the other showing the complete bridge
elevation. Supplemental photographs and sketches shall be provided, if necessary
to give a cleir understanding of severe bridge conditions.
Current ADT data shall be obtained using automatic counters provided by the con-
sultant. The counters shall be placed as necessary and operated for a periQd of
48 hours. ADT counts shall be adjusted using factors provided by DOH.
Inspectors shall meet t~e qualifications as stated in the National Bridge In-
spection Standards (NBIS)(23 .·C.F.R. Part 650).
Structural Analysis: Each bridge shall be rated in accordance with the Manual
for Maintenance Inspection of Bridges and AASHTO Specifications. The ratings
shall include ··the posting requirements for the three Colorado load posting vehicles .

Th e bridge materials shall be investigated so that applicable stress data can be
used in accordance wf th the specification requirements.
Inventory: All inventory items shall be c~pleted and coded on code forms, DOH
Form No. :736 and OOH Form No. 739. This information is itemized on DOH Forms 421
and 422. and will be used to generate reports to FHWA. Therefore. this infonnation . .
must be complete and correct. If deficiencies are found in coding, etc. the
material will be returned to the consultant for ·correction.
Reports: Inspection reports shall be furnished as follows:
One report for the city or county to include:
1. City/County Survey Map locating all bridges that were
inventoried within the City/County.
2. The following items shall be submitted by Structure Number:
A. Structure Inventory and Appraisal Sheets.
8. Supplemental Sketches. Photos, etc.
C. Rating Calculations and Summary Sheet.
D. Load Posting Requirements.
E. Code Sheets for Sl&A Data.
One report for the city or county to be given to the State to include:
1. City/County Survey Map locating all bridges that were
inventoried within the City/County.
2. The following items shall be submitted by Structure Number:
A. Structure Inventory and Appraisal Sheets.
B. Code Sheets for Sl&A Data.
C. Rating and Load Posting Sunmary Sheets.

All inspection reports are due by the time set for completion of the work in :the
contract.
Final Review: A final revfew will be held when 'all inspection work is completed
and reports hAve been furnished. This will be a joint rev·iew with City or County
officials, Staff Brfdge Design representatives, and the consultant to discuss the
inspection reports and answer any pertinent questions by either party. This re-

(1) Withholding of payments to the Con-
tractor under the ~ontract until
the Contractor complies, and/or;
(2) Cancellation, termination or sus-
pension of the contract, in whole
or in part.
F. Incorporation of P rovisions. The Contractor will
include the provisions of Paragraphs A through F
in every subcontract, including procurements of
materials and leases of equipment, unless exempt
by the Regulatio'ns, orders, or instructions issued
pursuant thereto. The Con ractor will take such
action with respect to any subcontract or procure-
ment as the State or the FHWA may direct as a means
of enfo rcing such provisions including sanctions
for noncompliance; provided , however, that, in the
event the Contractor becomes involved in, or is
threatened with, litigation with a Subcontractor
or supplier as a result of such direction, the
Contractor may request the State to enter into such
litigation to protect the interests of the State
and, in addition, t he Contractor may request the
FHWA to enter into such litigation to protect the
interests of th United States.

WHEREAS, the parties hereto desire to agree upon the division
of responsibilities with regard to this project.
HOW, THEREFORE, it is hereby agreed that:
A. The State will provide liaison with the Local Agency
through the State's Bridge Engineer, located at 4201
E. Arkansas Ave., Denver, CO 80222. Said Bridge
Engineer will also be responsible for coordinating
the State's activities tmder this contract.
B. The Lo cal Agency may enter jnto a contract with a con-
sultant to do all or a~! portion of the work unde~ the
pro pose d project; provided, however, that when Federal-
aid funds provided herein are to participate in the cost
of wo rk done by a consultant, the contract shall comply
with the requirements of FHPM, Volume 1. Chapter 7,
Sect ion 2, Administration of Nep,otiated Contracts.

The contract with the consultant
•hall be conditioned upon an acceptable proposal sub-
atantially in accordance with "Scope of Work" (Exhibit A)
attached hereto and made a part hereof and may not be
awarded or executed until the selection of the consultant
end the terms of the contract shall have been approved by
the State. All changes in the contract shall be by app-
plemental written agreement and must have prior approval
of the State. Ar. soon as the contract with the consultant ·· "
has been awarded, 3 copies of the executed contract shall

C. The Local Agency shall be responsible for the adminis-
tration of the consultant's work for the project. with
periodic inspections by the State and Federal Highway
Administration (FHWA). Administration shall include.
but not be limited to, monitoring the field inspections,
ratings and inventory data, prepared by the Loca~ Agency's
consultant, for accuracy and completeness. '
D. tbe State will administer the Local Agency and Federal-
aid funds for this project in accordance with the
following provisions:
Costs incurred by the Local Agency
11le Local Agency will prepare and submit monthly
charges for the costs incurred relative to the
project . However, bill\ng for less than $5,000
shall not be submitted unless it is for the final
project payment . The Local Agency will prepare
project charges in accordance with the Statis
standard policies and procedures. The State will
reimburse the Loc al Agency for the Federal-aid
share of the pro ject charges following the State's

Scope of Wor«
General: This work shall include all personnel, materials and equipment to inspect,
rate, and inventory bridges that are located in the city or county. Special equip-
ment such as a hydraulic operated bridge inspection platform, when required. will
be paid for as stipulated herein. Bridge inspection reports shall be prepared and
furnished as specified in the scope of work.
Bridge Survey: A review of any existing bridge survey data such as .inspection re-
ports, plans, etc. shall be conducted at the beginning of the inspection program
with city/county officials. A numbering system for bridges, based on guidelines
furnished by the State, shall be established for the city or county. This numbering
system shall be approved by the State. The contractor shall develop a systematic
• plan to search out and inspect all bridges ~1ithin the area as.signed. A copy of
this plan shall be submitted to the State for information.
A county map shall be prepared that identifies each bridge by number and locates
each bridge to the nearest 1/10 mile from some identifiable highway feature.
Bridge Inspection: All bridges and approaches shall be given a thorough inspection
to provide infonnation for the inspection report. The physical inspection shall
be considered part of the work of this contract. Where critical structural com-
ponents are inaccessible due to high water, inspections shall be re-scheduled to
accomplish the work. If it is detennined that critical structural components arc
not accessible with typical inspection equipment, as determined by the contractor
and concurred in by the State, arrangements shall be made to obtain special in-
spection equipment. A supplemental agreement will be required prior to the con-
• tractor proceeding with the work.

The Co ntractor will be reimbursed for the •ctual cost of renting special inspection
equi pme nt, operator's salaries, pennits or incidental work involved in the procure-
men t or operation of the equipment.
The items to be inspected and reported for OFF-SYSTEM bridges are outlined in the
St ructure Inventory and Appraisal (Sl&A) Abridged (Cities and Counties), DOH Form
No. 421. The items to be inspected and reported for urban system bridges are out-
l in ed in the Structure Inventory and Appraisal, DOH Form No. 422.
The Department of Highways Coding Manuals, Structure Inventory Coding Guide for
Bridges Not On The Federal-Aid System and Structure Inventory Coding Guide, elaborate
on each item. All inspection work will be conducted in accordance with the Bridge
Inspectors Training Manual 70 .
A mi nimu m of t wo color photographs will be required for each bridge, one viewing the
to p of the bri dge looking down the roadway, the other showing the complete bridge
elevation. Supplemental photographs and sketches shall be provided, if necessary
t o give a c l ear understand i ng of severe bridge conditions.
Cu r rent AOT data shall be obtained using automatic counters provided by the con-
sultant. The counters shall be placed as necessary and operated for a periQd of
48 hours . ADT counts shall be adjusted using factors provided by DOH.
Inspectors shall meet the qualifications as stated in the National Bridge In-
spection Standards (NBIS)(23 C.F.R. Part 650).
Structural Analysis: Each bridge shall be rated in accordance with the Manual
for Maintenance Inspection of Bridg es and AASHTO Specifications. The ratings

The bridge materials shall be investigated so that applicable stress data can be
used in accordance with the specification requirements.
Inventory: All inventory items shall be c~mpleted and coded on code fonns, DOH
Form No.:736 and DOH Form No. 739. This infontJation is itemized on DOH Forms 421
and 422, and will be used to generate reports to FHWA. Therefore, this infonnation
must be complete and correct. If deficiencies are found in coding, etc. the
material will be returned to the consultant for correction.
Reports: Inspection reports shall be furnished as follows:
One report for the city or county to include:
1. City/County Survey Map locating all bridges that "'ere
inventoried within the City/County.
2. The following items shall be submitted by Structure Number:
A. Structure Inventory and Appraisal Sheets.
B. Supplemental Sketches, Photos, etc.
C. Rating Calculations and Summary Sheet.
D. Load Posting Requirements.
E. Code Sheets for SI&A Data.
One report for the city or coilnty to be given to the State to include:
1. City/County Survey Map locating all bridges that were
inventoried within the City/County.
2. The following items shall be submitted by Structure Number:
A. Structure Inventory and Appraisal Sheets.
